Why does crisp bread have holes?

The holes are called docking. The purpose of docking is to keep the crust and bottom of crisp bread together. Without docking crisp bread turns out to be like a big ball.  Docking is also decoration of bread.

What foreign objects can be found in food?

Foreign objects get into foodstuffs accidentally e.g. due to environmental pollution or spoilage. For example pesticides are counted as foreign objects. Foreign objects found in food have accepted maximum amounts controlled by authorities.
